The Rubin Center is a private self-pay practice not in-network with any insurance companies. However, if you would like to contact your insurance company in advance and request an out-of-network provider exception, we will provide you with the information for Dr. Rubin regarding our private practice office to discuss with them to determine if your policy offers any benefits or reimbursement that may apply to your visit with Dr. Rubin visit . We are self-pay at the time of the appointment and will provide you with a superbill after your visit with Dr. Rubin with diagnoses and codes related to your visit to submit to your insurance carrier. You would want to request a review of these benefits prior to your visit with Dr. Rubin so they can document your request and prior review. Reimbursement is not guaranteed and based on your specific policy under your medical benefits.

We will provide you a Superbill after your evaluation appointment which contains all of the information needed to submit your claim to your insurance company. You are responsible for submitting your Superbill and seeking reimbursement. Although we do not communicate with Insurance companies, we are happy to answer any questions you might have by email or phone.
